We appreciate the opportunity to provide you with health care. Our staff is made up of well-qualified professionals, who work together as a team to bring you the highest-quality treatment in a warm, caring setting.
Hours of Operation:
Monday – Friday: 9am – 5pm (closed between 12pm-1pm for lunch)
We try to see all patients on an appointment basis and we request that you call in advance so that we may reserve time for you. We make every effort to honor all time commitments and request that you extend the same courtesy to us. If you cannot keep an appointment, please notify us immediately. We ask that you give us 24 hours notice. This courtesy makes it possible to give your appointment time to another patient who desires to see the doctor.
As a service to our patients, we will file your insurance for you with payment authorized to the physician. Any money received, will be applied to your account balance.
Copayments and deductible should be paid at time of service.
We wish to stress, THE FINANCIAL RESPONSIBILITY FOR SERVICES RENDERED RESTS WITH THE PATIENT OR HIS FAMILY, REGARDLESS OF INSURANCE COVERAGE.
Any financial benefits you may receive from your insurance are strictly a matter of settlement between you and that insurance company.
